    Appointments for Passport Services

    Passport services are back! We will be accepting appointments starting Tuesday, July 15th, 2025.

    The Coventry Public Library takes passport acceptance by appointment only.  If more than one person in a family is applying at the same time only one appointment is needed, up to 5 people. Please call the Reference Desk at 401-822-9105 to book an appointment or if you have any questions.
     

    • What do I need for my passport appointment?

Proof of US Citizenship An original or certified copy of your Birth Certificate or a Naturalization Certificate; or an expired US passport. Be sure the birth certificate is certified (embossed or with a seal, and on anti-copy paper) and includes a filing date of less than a year from the date of birth.
      Black-and-white outline illustration of a driver's license 2. Valid ID A current, valid photo ID (driver’s license or state ID) is the best ID. If your photo ID is from outside the state of Rhode Island, you will need a secondary form of identification, such as a student ID

      3. One professionally developed passport photo This site CANNOT take passport photos. Check here for current guidelines on acceptable passport photos.

Payment for the US Department of State - CHECK OR MONEY ORDER ONLY The US Department of State ONLY accepts checks or money orders. The Department of State does NOT accept cash or debit/credit cards for first-time passport services. If a website is asking for your debit/credit card number in exchange for first-time passport services, it is highly likely to be a scam. See the current fee schedule attached on the right side of the screen, or call the library for current rates. Note that adults and minors under age 16 are charged different rates.

      NO CARDS
      There is a mandatory fee of $35 per application for passport acceptance. This payment is separate from your payment to the US Department of State. The library accepts payment by check, money order, or cash. Checks should be made out to the Town of Coventry.

Forms can be picked up prior to your appointment at the reference desk, or filled out and printed from the online form here. Please use BLACK INK ONLY to fill out the forms and DO NOT sign until you are asked to do so by the acceptance agent. Depending on your circumstances, you may also need supplemental application materials. If you have any questions about what forms you need, call the library at (401) 822-9105.

       

      • What if I am applying for my child (age below 16)?

        If you are applying for a minor (under 16 years of age), you will need all of the above in addition to the following:

        Applicant Although minors under 16 years of age cannot sign for their own passport applications, they must be present for the appointment. For more information about applying with/for a minor, please check here.

        Proof of parental consent

        Either:

        1. Both parents named on the birth certificate, adoption decree, or court order must be present for the passport appointment with photo ID, OR
        2. DS-3053 (Statement of Consent: U.S. Passport Issuance to a Child) must be signed in front of a notary by the non-applying parent, and the applying parent must bring that form and a photocopy of the front and back of the non-applying parent's photo ID, OR
        3. DS-5525 (Statement of Exigent/Special Family Circumstances for Issuance of a U.S. Passport to a Child Under Age 16) must be filled out and submitted with application.

        For 16-17 year old applicants Minors aged 16-17 are qualified for an adult passport (valid for 10 years) and only need one parent present to sign their application.
