Passport Acceptance Information and Appointment

    The Coventry Public Library takes passport acceptance by appointment only.  If more than one person in a family is applying at the same time only one appointment is needed. 

    Forms can be picked up prior to your appointment at the reference desk.  Please use BLACK INK ONLY to fill out the forms and DO NOT sign until you are asked to do so by the acceptance agent. 

    Please make sure to bring the following to your appointment:

    1. Proof of US Citizenship

          An original or certified copy of your Birth Certificate or a Naturalization Certificate; or an expired US passport. Be sure the birth certificate is certified and includes a filing date of less than a year from the date of birth.

    2. Valid ID

           A current, valid photo ID (driver’s license) is the best ID.

            If you do not have a driver’s license or state ID, you must have at least two forms of secondary identification. 

    3. One professionally developed passport photo.  This site CANNOT take passport photos.  

    4.   Payment for basic passport services. The US Department of State only accepts checks or Money Orders, please make check out to US Department of State.

    5.  The library's fee for processing is $35.00 per application. These can be by check or money order and must be separate check or money order.  The check for the processing fee should be made out to Town of Coventry.
